In order to help you be prepared for our interview We ask that you arrive to the interview prepared with the following:

• A completed CHL application form, a completed CORI request form and information for 3 professional references we can contact.  You can DOWNLOAD these form by clicking HERE.  All forms are part of this packet. 

• Please print the forms, complete and bring to your scheduled interview.    

• You will need to provide a copy of your photo ID to accompany the CORI request form, a copy of your highest degree and a copy of any professional licensure during your interview. 

• You may want to use mapquest or google maps to get directions to your interview, and remember to give yourself enough time to travel so you arrive on time.
